Forms in the enrolment app should be titled above text entry boxes for clarity of people submitting information and admins of the system

The text boxes should be labeled above with their respective fields to reduce ambiguity. This becomes particularly important when reviewing applicants information from other parts of the world where its not obvious what field belongs to what.

Additionally, it should be indicated in an obvious manner which are required.

side note: institutional email doesn't appear to be required, yet the api call failed with 400 when the field was left empty.
