URLMap fail to restore PATH_INFO and SCRIPT_NAME when app throws errors

This is not really a bug, but if a mounted PSGI app throws exceptions, URLMap fails to restore SCRIPT_NAME and PATH_INFO values, causing middleware like InteractiveDebugger's assets (css, js) not loaded correctly.
