layout problems with name generator on mobile small screen browsers

The check box for Refused lines up in the wrong spot on the iphone + Safari name generator page. There is one row with text entry box, then "+Add" button, then check box. The next row down has the "Refuse" text on its own. The check box should be next to the refuse text and not on the line above.

I'll check on different browsers / devices but this worked on my iphone with Chrome.
