MidiPerformer: enabled state of instruments in panel can get mixed up #71

Depending on the order in which instruments are added, the enabled state might not be for the right instrument

To reproduce, add an instrument with an index above 0 and then add one with 0.
Now, when selecting a MIDI output, instrument indices are mixed up...
