Background
Users will complete a quiz/risk assessment that grades them as High, Medium, or Low risk. They'll be given helpful links, suggestions, and plans based on their grade along with general COVID information as resources. Once completed they'll receive a completion badge (think "I voted" sticker"), and they'll have the ability to share a Bingo card on social with de-risking activities.
This issue will specifically cover the quiz results/recommendations. Some quick wires are below.
Broad Hypothesis
We believe that creating an intake form that identifies an individual's risk factors and gives them resources for Community leaders who reach out to their communities and the younger generation within multi-generational family homes who have an at-risk family member will result in increased awareness and adoption of tracing efforts. We’ll know we’re right when we see a 60%+ level of tool *adoption; individual use of the intake tool and a decrease of COVID-19 transmission by half.
