Background

Users will complete a quiz/risk assessment that grades them as High, Medium, or Low risk. They'll be given helpful links, suggestions, and plans based on their grade along with general COVID information as resources. Once completed they'll receive a completion badge (think "I voted" sticker"), and they'll have the ability to share a Bingo card on social with de-risking activities.

This issue will specifically cover the quiz results/recommendations. Some quick wires are below.

Broad Hypothesis

  1. We believe that creating an intake form that identifies an individual's risk factors and gives them resources for Community leaders who reach out to their communities and the younger generation within multi-generational family homes who have an at-risk family member will result in increased awareness and adoption of tracing efforts. We’ll know we’re right when we see a 60%+ level of tool *adoption; individual use of the intake tool and a decrease of COVID-19 transmission by half.

  2. We believe that creating a helpful how-to guide on preventive measures residents can take to avoid the contracting and transmission of COVID-19 for Community leaders who reach out to their communities and the younger generation within multi-generational family homes who have an at-risk family member will result in increased awareness and adoption of tracing efforts. We’ll know we’re right when we see a 60%+ level of tool *adoption; individual use of the intake tool and a decrease of COVID-19 transmission by half.

Scenario: A user completes their quiz for recommendations

  • When I start the quiz
  • Then I can complete a series of questions about my risk factor
  • When I complete those questions
  • Then I cam given recommendations based on my answers

Here's where I'm at:

  • colors are somewhat placeholder, taken from Louisiana dept of health
  • font is placeholder (wanted to see if the logo would inspire a font choice, also may want to steal from the landing page, which is OpenSans + Montserrat)

Things I that need more attention:

  • logging COVID19 status (part 1 of the quiz flow or maybe a separate step altogether)
  • what logging a positive or negative status looks like afterward (negative status = take risk assessment anyway, positive = we sorry, here's some resources)
  • want to make the anonymous nature of this app more apparent. May steal verbiage from the mobile app.
  • Results screen (needs love, but wanted to see what the badge looks like and get an idea of what other info we wanna show or link to from that screen)

  • Like the color, good to start, can tweak later.
  • Getting you badge & then bingo card.
  • Can we use paired down LDH logo "+" in monochrome white on start screen FPO?
