We have #62 and possibly #71 in progress for getting a quick V1 Typeform survey out the door. We have been working with Veena Billioux to get this out the door as quickly as possible.
Once we make some final edits, Veena suggested we send the survey to the following people offering assistance and perhaps getting it embedded in the LA Dept public health site. Veena suggested, we send it for visibility and cc herself and Scott Teesdale.
She suggested the phrasing to be around getting this first version accessible to the public before moving to phase 2. It will not only point people to determine their own risk but also, secondarily serve as an educational tool (by reading the risks and symptoms), and give us insight into the data that comes back.
Once we have a working tool it needs to go to these people to be put on the OPH webpage and hopefully it can be highlighted in the press conference tomorrow (Wed 6/3). Scott, do you think it should go to the whole list below or just the first 4 names?

Background
