Let vm-loader generate index.js for those modules do not have it, in local mode

模块的js在被webpack加载时，local模式需要被嵌入代码，所以对于无js的模块，加载html时要和模块的js一样判断是local模式然后嵌入相同代码，这个过程vm-loader和index-loader是重复的。目前权衡的结果是，不在vm-loader中加入这个过程，希望模块开发者能保证给模块有一个index.js。
