How do you choose priorities?

With 1000+ public packages and probably some private projects, how do you organize your work time? Your day-to-day. How do you choose what to work on? Whether to work on new features, go back and fix some issues, review pull requests, answer questions or to start a new project. The complexity and amount of work probably grows exponentially with every new package you create.
