[Test Panel] auto-generated form used, static files unaccessible for certain forms

certain forms appear as the auto-generated version on the test panel, there is something with the system being unable to access the static files for these forms, causing this to happen.

To Reproduce
Steps to reproduce the behavior:

1. Go to an uploaded WorkBench data set
2. Click on the Results tab
3. Scroll down to Create Record Set
4. See error

This issue makes it very hard to accurately test on production.

The default view options when adding a new form definition in app resources aren't accurate to what a user would see, and send to 404
