Type of issue (form, required):
- Type of issue
- Downed tree
- Poor drainage/blocked culvert
- Hazardous erosion (such as a small landslide)
- Structure failure (such as a bridge or retaining wall)
- Missing/vandalized/badly damaged sign(s)
- Seasonal maintenance needed
- Other (Describe)
- Location on trail (auto)
- Where on trail (i.e. mile marker, autofill, form?)
- Additional comments
