Name for first heading

Which there is an option to take name from first heading instead of file name.
This is the setting that I use with IndexMenu or AlphaIndex plugins on Dokuwiki as it is a multilingual wiki using translation plugin which require file name to be the same across languages.

You can do this by including it in the link markup:
[[:servers|Web Servers]]
I created an example for you:

But there is no way for the plugin to update names, since the epub document is final once it is created.

Yes. I Know.
I wasn't enough specific as I don't set page manually but build it from the whole namespace. and so can't manually provide name.
The wiki will evolve a lot so the need of using the namespace option.
But page are stored with .md extension and filename can't always be the title because of the multilingual context.
