Picture Badge not working

Great extension, but when I try to either create or edit a user badge, and turn on "Badge is an image", the box disappears so that you can't see any of the settings. I did find that if I hover over where the box was, I can see that it is recognizing the buttons and text boxes, you just can't see them. I tried saving the changes that way, but was still unsuccessful in having an icon for the badge.
