Just the level id and DMs are used. This means if any 2 or more packs have a level with the same id there will only be one best time for 2 levels, yet the example pack sates that the level id should be unique per pack and not totally unique.
The level in the copied pack was not played, but does have a best score cause the level in the original pack was played.

This is definitely an issue with local profiles and their packs. With online leaderboards this is definitely not an issue, because as soon as you start changing the file contents of the level it'll immediately create a new leaderboard for that level.
