Improving handling of deleted source objects

Currently, the stored Document objects can get out of sync with the sources from which they are generated.
If a source object is deleted, the related Document may still be picked up by a similarity query.

This can likely be solved when implementing #6, but also need to consider how this is handled when automatic index management is not enabled.
