* #163 - splitting code table 4.2 into its subtables, by disciplines and categories.

* xml,txt files

* delete GRIB2_CodeFlag_4_2_CodeTable_en.csv as it is now available as sub-tables

* xml,txt files

* update script create_master_lists to cope with new Code Table 4.2 subtables

* xml,txt files
