User feedback form

Katie Walker has requested the ability to gather user feedback after they've used YCB. More work needs to be done in collaboration with Katie to define what fields we would gather from users, but I would recommend we do something simple to start, such as using a Google Form. Then, if Katie finds it to be useful, we think about integrating the form into the site.

Katie has created the form she wants here. If we could convert this to a Google Form, we could have a link show up after users fill out the survey.

When I tried transferring ownership to you, I got the message

"Sorry, cannot transfer ownership to [email protected]. Ownership can only be transferred to another user in the same domain as the current owner."

Is there a way around this? The only thing I can think of is to re create or make a copy of the form under your domain.
