Subflow in actionable notification does not return any action

i can see the event and the action in event listener in ha.
Tried putting a debug in but it does not display
Ha on phone is work fine message appears hold done message to get actions select yes, even takes me to web page
using iphone

i have taken the event section out of the subflow to debug, and now can see "TypeError: Cannot set properties of undefined (setting 'eventData')" on the build message section

looking at the listener in ha {

Assuming something has changed? Any idea? how to fix
