This PR reorganizes the documents directory with subdirectory for rev 4 and moves the appropriate files there. This will allow for separate but ongoing maintenance of rev 4 documents (guides & extension docs), and the addition of a separate rev 5 subdirectory when those documents are ready for release. NOTE: This PR includes minor link updates in the FedRAMP OSCAL Vendor Resources PDF to point to the correct rev 4 version of the FedRAMP OSCAL guides.
