Multiple Reportlets in Report Form

This adds dynamic Reportlet fields to the Report Form so multiple Reportlets can be included in one Report.

This is not final and needs to be implemented in a more standardized manner.

Depends on:

• Add Collection for dynamic Forms ipl-html#71
