I've elaborated a very simple template that can be used for bug report.
Might be a good start, hope it helps.

System Info (Blender >> Help >> Save System Info):

Observation: On the Blender Menu Help >> Save System Info, saves a info file that might help to see if other addons interactions might be causing issues. This log shows all the addons at the end of the file.
