This pull request updates the NCD intake form to match the current paper version. The changes include:

Updating field labels and input types to align with the paper form.
Improving the layout for better usability and consistency.
Revised validation rules to meet the latest data requirements.
Fix any bugs identified in the form submission process.

Only two sections were replicated from the SL intake form (types of visits and vital signs).
