Sorry for the delay for reviewing the pull request 😓
I have added 2 comments about the configuration of the issue form, nothing serious.
It is a really good start for our small repository.
Regarding the description of the #21 issue, I think you did not manage the second and third points. Do you need some help for these ones?
- Fixing wrong spelling of "Steps to reproduce" - Deleting field for contact information
