Add new field in booking form and add new icons on it and also improve css

Description

In previous code after submit booking form our website is not work because path is not set after submit .
I have done following thing

1. Add new filed in booking form
2. Add new icons for each filed
3. Improve css
4. Add redirect link after form submit
5. Custom message after submit
