Improve/fix logic for several things around loading submenu items

  • Fix debugger provider and remove the "updateSubmenuItems" message
  • Change "refreshSubmenuItems" message to accept specific providers (or dependsOnIndexing or all)
  • Fix submenu logic causing it to load open files too frequently
  • Fix clipboard cache containing duplicate items
  • Fix logic checking if open files have changed
