  1. disasm: add ir3_assert() 

    Sometimes crashdec wants to try to disassemble something that *might* be
valid instructions.  We want to give it a way to recover if that turns
out to be a bad assumption.

  2. crashdec: override ir3_assert handler to recover from disasm errors 

    Sometimes things we want to try to decode as shader instructions, aren't
actually valid shader instructions.  So provide our own assert handler
to recover.
