The difference is the resolution. The original game has a resolution of 800x600 that looks bad on modern 16:9 displays due to black bars.
This patch contains upscaled game elements (such as BG's, CG's, sprites, interface elements, etc.) so you can install it and enjoy a visual novel "Katawa Shoujo" in high definition.
You have to download the latest release from the "Releases" section, unpack the .zip folder wherever you want and open "setup.exe".
Then follow the instructions there.
Click RMB on your "Katawa Shoujo.exe" and select "Properties". Then go to the Compatibility tab and click "Change high DPI settings".
And in this menu turn all checkers on (like on screenshot below), click "OK".

Just open the save and overwrite it.
But to avoid any problems with saves after installing the patch, I highly recommend starting a new game and skip read text to the moment your previous save was at. "How to turn on Skip mode" is written in Game Manual.pdf in your game folder.

You probably launched an old save of yours. Unfortunately, the old saves contain the old settings and the old issues, so you have to start a new game to make the issue disappear. But firstly, make sure your patch version is 1.1.3 or above.
If it doesn't work: go to /KSHD/game/ui/, delete file "ctc_strip" and rename "ctc_blank" to "ctc_strip" (just like the file you've deleted). The problem will disappear, but so CTC-arrow will.

If it still doesn't work: I can only suggest you move the persistent file to some other folder. The game will create a new persistent file and the issue can be solved. But if not, you could return your progress by returning the old persistent file back to the folder.
(Path to file: C:\Users\[YOUR_USERNAME]\AppData\Roaming\RenPy\katawashoujo_actual_1.3\ for Windows Vista/7/8/10)
