A few notes:

• Ask something about user's background and position, e.g., are they PhD student, postdoc, professor, analyst in control programme, ...
• Hope to get ideas for prioritisation of both new training materials and API functionality.

I have been working on a first draft here, it would be cool to get folks (@alimanfoo, @ahernank, @leehart , @jonbrenas, @KellyLBennett) thoughts on what is missing or should be removed. I have some edge case stuff in the SANDBOX section at the bottom.

We will need to offer at least an English and French version of the survey.

We can use Google Forms, Paballo will help setup when he returns.
