Updated samplesheet to include 'address' column

Changed input samplesheet to include:

sample	mlst_alleles	address
sampleQ	.../sampleQ.mlst.json
sample1	.../sample1.mlst.json	1.1.1

The address column can be left empty: will be considered null in pipeline and will trigger a query.

[Removed the profile_type, which was a required boolean value]
