  1. Add failing tests for prolog variations 

    Adds fixtures to test:
 - Missing optional XML declaration (currently works, misses a test)
 - Missing optional XML declaration with a standard '<?xml-' Processing Instruction (Fails)
 - Missing optional XML declaration with a custom '<?' Processing Instruction (Fails)
