P5 ENV template update (for P5R) (#75)

* Update p5_env.bt

Added new field names for identified fields in P5R - updated addresses to match names where there was mis-alignment. For fields with multiple names, slashed them where there is ambiguity. 

Could potentially mark unused fields, but have not done so yet.
