CARD-2043 Apex REST call failing in orgs with the url <>.visualforce.com

Fixing logic issue when creating instance url when url is visualforce.com.

@psubramaniam46 Can you speak to Sachin please - he's been investigating a similar issue with forceTK and the code you have looks like it might fail for similar reasons as he's hit.

Hi Matt, I have checked with him, the issue that he was facing is different the url which gets generated by the logic is different from the request url in his case. Thanks!
