Refactored unique name_id handling

⚠️ Breaking changes in 0.14.0 ⚠️

• updated API Controller,
• Tree model
• db table structure

A name_id will now be generated from domain_id + access_domain which is a unique database index

name_id is further available as an virtual model attribute
